Privacy Policy

Last updated: July 27, 2026

Note: This policy is written to accurately describe how this website actually operates — the forms, cookies, and third-party tools in use — but it is not a substitute for review by a licensed attorney familiar with insurance-marketing and data-privacy law in the states Cohen Nationwide Health Insurance Advisory operates in. Have counsel review this before relying on it as your final, published policy.

Cohen Nationwide Health Insurance Advisory (“Cohen Nationwide Health,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through https://cohennationwidehealth.com (the “Site”), why we collect it, how we use and protect it, and the choices you have. By using the Site, you agree to the practices described here.

1. Information We Collect

Information you give us directly

When you submit a quote request, contact form, referral form, or portal sign-in, we may collect your name, phone number, email address, ZIP code, household size, approximate income range, current coverage status, and any notes or messages you choose to include. If you book a consultation, Calendly collects the scheduling details you provide there under its own privacy policy.

Information collected automatically

Like most websites, we automatically collect certain technical data when you visit: IP address, browser and device type, pages viewed, referring URL, and approximate location derived from your IP. We also use cookies and similar technologies (see Section 4) to support analytics and advertising measurement.

Information from other sources

If you are referred to us by an existing client through our referral program, the referring client provides us your name and contact information so we can reach out about coverage options.

2. How We Use Your Information

We use the information collected to:

  • Respond to your inquiry and provide insurance quotes, comparisons, and guidance;
  • Contact you by phone, text/SMS, or email about the request you submitted;
  • Schedule and manage consultations;
  • Operate the client and agent portals, including processing referrals;
  • Measure and improve the performance of our marketing (analytics and ad conversion tracking);
  • Comply with insurance licensing, recordkeeping, and other legal obligations; and
  • Detect, prevent, and address fraud, abuse, or security issues.

We do not sell your personal information, and we do not share it with unrelated third parties for their own marketing purposes.

3. Consent to Be Contacted (Important)

By submitting a form on this Site with your phone number, you expressly consent to be contacted by Cohen Nationwide Health Insurance Advisory and its licensed advisors by phone call, SMS/text message, and email — including by automatic telephone dialing systems or prerecorded/artificial voice where applicable — regarding insurance products and services, even if your number is on a state or federal Do-Not-Call registry. Message and data rates may apply. Consent is not a condition of purchasing any product or service. You may revoke consent at any time by replying “STOP” to any text, telling us during a call, or emailing kenazcohenhealth@gmail.com.

4. Cookies & Tracking Technologies

We and our service providers use cookies, pixels, and similar technologies for:

  • Essential functionality — keeping the Site and forms working correctly.
  • Analytics — Google Analytics (GA4), Google Tag Manager, Vercel Analytics, and Microsoft Clarity, to understand how visitors use the Site.
  • Advertising — Google Ads and Meta (Facebook/Instagram) pixels, to measure ad performance and, where enabled, show relevant ads.

Most browsers let you block or delete cookies in your settings, and you can opt out of interest-based Google and Meta advertising through Google Ads Settings and the Digital Advertising Alliance opt-out page. Blocking cookies may affect how parts of the Site function.

5. Third-Party Service Providers

We share information with the following categories of service providers, solely to help us operate the Site and serve you:

  • Supabase — secure database hosting for lead and referral records, and portal authentication.
  • Resend — transactional email delivery (lead and referral notifications).
  • Calendly — consultation scheduling.
  • Vercel — website hosting, performance, and analytics infrastructure.
  • Google (Analytics, Tag Manager, Ads) and Meta — analytics and advertising measurement.
  • Insurance carriers — when you move forward with an application, relevant information is shared with the carrier(s) you choose to apply with, as required to quote and enroll you in coverage.

We may also disclose information if required by law, subpoena, or regulatory request, or to protect the rights, property, or safety of Cohen Nationwide Health, our clients, or others.

6. Data Retention

We retain lead, client, and referral records for as long as needed to provide our services and to meet insurance recordkeeping and licensing requirements, after which we delete or anonymize the data unless a longer retention period is required by law.

7. Data Security

Form submissions are transmitted over encrypted HTTPS connections and stored in an access-controlled, encrypted database. Portal accounts use passwordless, one-time-link sign-in rather than stored passwords. No method of transmission or storage is 100% secure, and we cannot guarantee absolute security.

8. Your Rights and Choices

Depending on your state of residence, you may have the right to request access to, correction of, or deletion of your personal information, to opt out of certain marketing communications, and — for California residents under the CCPA/CPRA — to know what categories of personal information we collect and to request deletion. We do not sell personal information as defined by these laws, so there is nothing to opt out of in that respect. To exercise any of these rights, email kenazcohenhealth@gmail.com or call (786) 886-8364; we will verify your identity before fulfilling data requests.

9. Children's Privacy

This Site is intended for adults seeking insurance information and is not directed at children under 18. We do not knowingly collect personal information from children.

10. Changes to This Policy

We may update this Privacy Policy from time to time. The “Last updated” date above reflects the most recent revision. Continued use of the Site after changes are posted constitutes acceptance of the updated policy.
