PPO stands for Preferred Provider Organization. It's a network of doctors and hospitals that agreed to discounted rates with your insurance carrier — but unlike an HMO, a PPO doesn't require you to get a referral before seeing a specialist, and it provides at least partial coverage if you go outside the network.

How the network works: 'in-network' providers have a negotiated rate with your carrier, so your out-of-pocket cost is lower and predictable. 'Out-of-network' providers haven't agreed to those rates, so you'll typically pay more — sometimes significantly more — and your plan may only cover a percentage (coinsurance) after a separate, higher out-of-network deductible.

National PPO networks like Choice Plus PPO and First Health PPO are used by multiple carriers and offer very broad access — often useful if you split time between states, travel frequently for work, or simply want the reassurance that most major hospitals and specialists nationwide are covered.

What you'll actually pay: premium (monthly, regardless of usage), deductible (what you pay before insurance starts covering costs), copay (a fixed fee for a specific service), and coinsurance (your percentage share of costs after the deductible, until you hit your out-of-pocket maximum). PPO plans typically have a higher premium than an equivalent HMO but lower total cost if you see specialists often or want out-of-network flexibility.

When a PPO makes the most sense: you have an existing relationship with a specialist you don't want to lose, you travel or live in multiple states, you want flexibility to see any doctor without navigating a referral process, or you're generally healthy and want to minimize the odds of an unexpected out-of-network bill.
